PCAB Accreditation Report Summary

Patient, physicians, nurses, and others are invited to locate a PCAB Accredited compounding pharmacy and review its PCAB Accreditation Report Summary by clicking on the "PCAB Accredited Seal" below. Each accredited pharmacy has a printable Accreditation Report posted and arranged by state. Each report lists the scopes of compounding practice performed by the pharmacy at the time of the latest Review and Survey. Some PCAB Accredited compounding pharmacies practice on all areas, some specialize in one or two. It is important to know your pharmacy's Scope of Compounding Practice [PDF file]. The four scopes of compounding practice recognized by PCAB are:

Non-sterile basic
Non-sterile complex
Sterile—low/medium
Sterile—high

How to Read the Accreditation Report Summary

The Compounding Scope of Practice Report provides several bits of information regarding the pharmacy's accreditation. It indicates the accreditation status of the pharmacy. In addition, the Compounding Scope of Practice Report gives the date on which the pharmacy was first accredited, the date of the last review (a documentation and self-inspection annual review); the date of the last survey (an in-pharmacy inspection); and the next scheduled review and survey.

Also provided is information regarding the pharmacy's compounding scope of practice. Every person using this Accreditation Report should review the Compounding Scope of Practice Report and the explanation page. The reader should review the scope of compounding practiced by the pharmacy on the date of the last in-pharmacy survey. Not every pharmacy will, or should, practice all scopes of compounding. Many pharmacies specialize in one or two areas of practice. Some pharmacy practice in all four areas or scopes of compounding.

A pharmacy may change its scope of compounding after the survey. If it does so, it must report the changes must be in accordance with PCAB Rules to which the pharmacy agreed at the time it applied for accreditation. The rule governing changes, states:

If at any time an Accredited Pharmacy expands its Compounding Scope of Practice, the Accredited Pharmacy shall:

Meet the Standards applicable to the expanded Compounding Scope of Practice prior to dispensing;

Notify PCAB™ within thirty (30) days of expanding;

Submit an abbreviated application within sixty (60) days of expanding its Compounding Scope of Practice; and

Be fully accredited by PCAB™ for its entire Compounding Scope of Practice within six (6) months.

Following the Compounding Scope of Practice Report is the Summary Report, a one page, abbreviated report. A full report follows the Summary Report.
