The information we collect about you on our website is used in the accreditation process and provide a more personalized navigation through our site. All data collected is handled as “strictly confidential” in accord with the Confidentiality Statement below, and is never accessed without the pharmacy’s express permission. Confidentiality
All information PCAB obtains through the application, evaluation and certification process shall be deemed confidential. By default, your login password is used as your encryption key. PCAB agrees that the information in the application or that PCAB obtains shall be used for the purpose of evaluating the applicant’s eligibility for PCAB qualification. We will not sell or provide your information to others except through our website as set for in the terms and rules to which you must agree prior to submitting your application.
Information provided in the application or obtained by PCAB is in the public domain. PCAB reserves the right to notify all appropriate regulatory or law enforcement authorities when PCAB, its employees, or agents believe in good faith that the pharmacy, the corporation, the owners, the corporate staff, and/or the pharmacy staff are engaging in conduct that violates state or federal law. When requested PCAB or any of its employees or agents will disclose the information through any governmental administrative or judicial process.
Passive Information Collected
Our site may utilize a standard technology called "cookies" to collect information about how this site is used to enhance the on-line accreditation process. We may use Passive Information gathered to enhance the design of this site to make it easier, faster and friendlier to use.
